No puede pronosticar el futuro

Una emergencia puede ocurrir en alguno minuto. Esta mañana como los residentes estuvieron preparando para sus rotaciones, de repente unas de sus compañeras se cayó y ella empezó tener una convulsión. Tan pronto como su compañera cayó al piso, los residentes y los médicos presentes en la sala tomaron acción. Todos trabajaron eficientamente. Algunos obtuvieron una camilla mientras otros pusieron la mujer en su lado izquierdo y trataron controlar sus convulsiones. Era un momento con mucho esteres, pero cuando los médicos resolvieron el problema, el ritmo del día regresó...

Observaciones
Colaboración
Los procesos de diagnosis y el tratamiento son muy colaborativos en el hospital de Córdoba. Por ejemplo, en el caso de los residentes y la médica que estoy observando, quieren consejo de un especialista sobre el paciente #18. Mientras su condición está mejorando, tienen preocupación sobre el derrame. Por eso visitamos otro piso del hospital para solicitar a la ayuda de un especialista. En basado de esta interacción y el reunión de ayer as obvio a mí que el hospital de Córdoba quiere proveer el mejor cuidado para sus pacientes. Los doctores de la clínica médica se reconocen que tienen una educación completo y a veces necesitan la ayuda de doctores con una educación más especializada para resolvar los casos complicados. No hay egos: todos los doctores funcionan como un equipo.

Electrocardiograma 
Hoy Bruno condujo un electrocardiograma para paciente #4. Quisieron monotorizar como su condición estuvo afectando la circulación a sus piernas. Bruno me enseñó la manera corecta poner los sensores en el cuerpo y conectar los cables al paciente y la máquina. Ayudé que el electrocardiograma usa los corrientes eléctricos para obtener información sobre la manera en que la corazón está bombeando el sangre por todo el cuerpo. El resultado para paciente #4 era normal. En contraste, un paciente que Carolina tiene fue ingresado a causa de un ritmo irregular. Ella me mostró el electrocardiograma original y había dos incidencias de intervalos irregulares de QRS (El "S" era muy larga). El electrocardiograma más actual era normal.

La sterilización y el higiene
Hoy, durante los examinacoines físicos en la sala de 105 que contiene pacientes #17 y #18, uno de los residentes usó guantes para examinar las piernas del paciente #17 y entonces él procedió completar el examen físico de paciente #18 sin cambiar sus guantes. Adicionalmente, por todo el día la doctora y los residentes nunca lavaron sus manos o usaron el liquído higiénico. Mientras ninguno de los pacientes tienen condiciones contagiosos,  pienso que más precauciones and limpieza habría tenido occurir en los EEUU, especialmente a causa de la paranoia aguzado de la difusión de enfermedad. La única vez que la sterilización ocurrió fue durante el electrocardiograma. El residente limpió los sensores que se puso en el paciente y el residente puso el alcohol para fricciones en las piernas y las muñecas dle paciente. Sería interesante saber la prevelancia de condiciones como MRSA y otras condiciones transmitadas normalmente en los hospitales.

Una exploración de la ciudad

Cerca del centro de la ciudad y la plaza de San Martín hay un peotonal grande. A causa de nuestra reunión en la oficina de ICC fue cancelado, decidió explorarlo. Las calles están cubiertas con tiendas de la ropa, los electrónicos, los accesorios y los restaurantes. Entre todas las personas caminando por todo el peotonal había artistas de la calle y los venedores ambulantes también. Es claro que los zapatos de plataformas son populares~en Argentina porque los estantes de cada zapatería fueron llenos de varios estilos de esos zapatos. Durante esta exploración me encontré mi primero regalo de este viaje!

An emergency can happen at any minute. This morning while the residents were preparing for their rotations, suddenly one of them fell and she began to convulse. Observations
Collaboration 
The diagnosis and treatment processes are very collaborative at the Córdoba hospital. For example, El proceso de diagnosis y el tratamiento es muy colaborativo en el hospital de Córdoba. For example, in the case of the residents and the doctor that I am observing, they want advice from a specialist about patient #18. While his condition is improving they are concerned about the hemorrhage. So, we visited another floor of the hospital to ask for help from a specialist. Based on this interaction and the meeting from yesterday it is obvious to me that the Córdoba hospital wants to provide the best care for their patients. The doctors of internal medicine recognize that they have a comprehensive education and sometimes they need help from the doctores with a more specialized education in order to solve challenging cases. There are no egos: all of the doctors function as a team.

Electrocardiograma 
Today Bruno conducted an electrocardiogram on patient #4. They wanted to monitor how his condition was affecting the circulation to his legs. Bruno taught me the correct way to put the sensors on his body and connect the wires from the patient to the machine correctly. I learned that the electrocardiogram uses electric current to obtain information about the way in which the heart is pumping blood throughout the body. The results for patient #4 were normal. In contrast, a patient that Caroline has was admitted because of a irregular rhythm. She showed me the original electrocardiogram and there were 2 isolated  irregular QSR intervals ("S" was very long) The most recent electrocardiogram was normal.

Sterilization & Hygiene
Today, during the physical examinations in room 105 that contains paitents #17 & 18, one of the residents used gloves to examine the legs of patient #17 and then he proceeded to complete the physical examination for patient #18 without changing his gloves. Additionally, throughout the day the doctor and the residents never washed their hands or used hand sanitizer. While none of the patients had contagious conditions, I think more precautions and cleaning would have taken place in the United States. Especially because of the heightened paranoia of the spread of disease. The only time that sterilization occurred was during the electrocardiogram. The resident cleaned the parts of the body that he put the sensors and he put rubbing alcohol on the legs and wrists of the patient. It would be interested to know the prevalence of conditions like MRSA and other illnesses commonly transmitted in hospitals.

An Exploration of the City

Near the center of the city and the San Martín plaza there is a large pedestrian mall. Because our meeting at the ICC meetting was cancelled, I decided to explore it. The streets are lined with clothing, electronic and accessory stores and restaurants. Among the people walking throughout the pedestrian mall there are street performers and peddlers also. It is clear that the platform shoes are popular in Argentina because the shelves of every shoe store were full of various styles of these shoes. During this exploration I found my first gift from the trip!
